Program Overview

This activity is designed to educate clinicians on how to reduce care disparities and health outcomes for Black Americans in rural Mississippi with a venous thromboembolism (VTE) diagnosis, as they make the transition from a Critical Access Hospital (CAH) back into their own communities on oral anticoagulants.

Target Audience

This activity is designed for multidisciplinary healthcare professionals (HCPs) practicing in CAHs in Mississippi, including specialists in vascular medicine, pulmonologists, vascular surgeons, hematologists, hospitalists, primary care providers, emergency department physicians, allied HCPs (ie, NPs, PAs, pharmacists) and/or other HCPs involved in the coordination of care between practice settings for patients with VTE.

Learning Objectives

Upon completion of this educational activity, participants should be able to:

• Discuss disparities in care among Black Americans with VTE, including disparities in receiving direct-acting oral anticoagulants
• Describe real-world evidence on the racial and ethnic disparities in the presentation of COVID-19, the hypercoagulability state of patients, and key practice considerations
• Implement optimal approaches to the recognition and treatment of VTE with direct-acting oral anticoagulants, including tools and resources to engage patients in their care plan
• Evaluate how a “bridging” strategy with both pre-discharge and post-discharge interventions can improve care transitions for Black patients with VTE
• Provide effective, sustainable, and culturally appropriate interventions to improve outcomes through optimal care transitions that reduce unplanned CAH readmissions and health disparities for Black patients with VTE in Mississippi CAHs

Dr. Charles Pollack earned his medical degree from Tulane University School of Medicine. He is currently a Clinician-Scientist in the Department of Emergency Medicine at the University of Mississippi School of Medicine. Dr. Pollack has long been active in teaching and clinical research and is an international leader in emergency medicine. He is the only physician to have received the American College of Emergency Physicians’ highest national awards in both teaching and research; he also received the national teaching award from the Council of Emergency Medicine Residency Directors. In addition, Dr. Pollack is the only US emergency physician to be elected a Fellow of the European Society of Cardiology. His primary research interests are in the management of thrombosis, reversal of anticoagulation, and other aspects of critical care.

Dr. Pollack has written more than 500 original research articles, chapters, and abstracts, and serves on the editorial boards of several journals and is on the steering committees of multiple national and international studies. He is the principal investigator on multiple ongoing trials and studies. He is also a strong advocate for entrepreneurism and innovation in healthcare. Dr. Pollack is a founding board member of the Hospital Quality Foundation, a non-for-profit education and research organization dedicated to improvement in the quality of care provided to patients in the hospital and at transition back to the outpatient setting.

DeGail J. Hadley, DO

Dr. DeGail Hadley received his medical degree from Kansas City University of Medicine and Biosciences. He completed his residency in family medicine at McLaren Macomb Hospital in Mount Clemens, Michigan. Dr. Hadley practices family medicine and is the Founder and Chief Executive Officer of Dynamic Wellness Medical Clinic located in Cleveland, Mississippi. He was drawn to the field of family medicine by the fact the field is diverse. As an Osteopathic Primary Care Physician, he treats a wide variety of illnesses that affect the entire family. Dr. Hadley is a community advocate and influential African American physician whose focus is on educating his patients and developing a healthier community, one patient at a time. He is blessed to practice in his hometown, which enhances his desire to provide thorough healthcare to his patients.

Dr. Hadley is the immediate past president of the Mississippi Osteopathic Medical Association (MOMA). He has lectured on diabetes, hypertension, COVID-19, healthy men, and obesity, just to name a few. Since returning to the Mississippi Delta in 2012, he has served over 10,000 patients. He believes health is essential and one of the principles to a positive and productive life. Dr. Hadley is the recipient of several service awards (Health Care Hero Award, Business Person of the Year, 70X2020 Colon Cancer Initiative Community Award), just to name a few.

Dr. Cynthia Harrington received a master of science as a family nurse practitioner from Delta State University in Cleveland, Mississippi, and a doctor of nursing practice from the University of Alabama in Tuscaloosa. In addition to her post-graduate nurse educator courses at the University of Mississippi Medical Center, she earned post-master certificates as a nurse educator from Walden University and as a psychiatric mental health nurse practitioner from the University of Southern Mississippi. Dr. Harrington is American Nursing Credentialing Center (ANCC) dual certified as a family nurse practitioner (FNP) and psychiatric mental health nurse practitioner (PMHNP). She is currently a Certified Family Nurse Practitioner at the Louisville Medical Clinic and the Aberdeen Health Clinic in Mississippi, as well as a Nurse Educator at Aspen University, Denver, Colorado.

Dr. Harrington is a member of the American Diabetes Association, American Association of Diabetes Educators, American Association of Nurse Practitioners, American Nurses Association, American Nurses Credentialing Center, American Psychiatric Nurses Association, American Society of Clinical Pathologists, and the Emergency Nurses Association, among others. Her research interests include diabetes, hypertension, hyperlipidemia, renal disease, obesity, and breast cancer.

Dr. Elizabeth Hood received her PharmD degree from the University of Mississippi and completed a pharmacy practice residency at the University of Mississippi Medical Center (UMMC). She is a Clinical Pharmacy Specialist at the University of Mississippi Medical Center in Jackson, Mississippi. Dr. Hood serves as the Director of the Anticoagulation Clinic which cares for both adult and pediatric patients on anticoagulation therapy. Pharmacists in the clinic provide care for patients throughout the state, many with low incomes and no insurance, and facilitate medication access and education.

Dr. Hood co-chairs the Anti-Clot Committee (Anticoagulation and Thrombolytic Oversight Team) which oversees the systematic management of anticoagulants within the hospital and clinics. In addition, she works as the pharmacist for UMMC’s Advanced Heart Failure/Transplant Clinic. She serves as a clinical preceptor for the University of Mississippi (UM) School of Pharmacy, as well as residency preceptor for pharmacy residents with UMMC and the UM School of Pharmacy. She is a member of ACC and the Anticoagulation Forum.
